How Long Does it Take to Obtain the Certificate?
Applications for certificates are processed by the Government the
next working day and mailed out to the applicant the next day. You
can obtain the certificate the next day in case of an emergency.
Please check with us for details.
Certificates for events that occurred during the last thirty days may
not be available if the event has not been registered. It may take
up to one month for Vital Statistics to receive birth, marriage or
death information from hospitals, ministers and funeral homes.
You can still make the application, but the certificate will not be
issued til the event information has been received by
Vital Statistics.
Vital Statistics Certificate Application Forms
If you require a Birth, Marriage or Death Certificate, you may print
the following form. Read the instructions carefully and fill in the
required information. You can follow one of the following
procedures.
All Vital Statistics applications MUST be accompanied by a
copy of your valid identification (birth certificate, current
driver's license, passport or citizenship card) faxed to

Only a certified photographic print and medical certificate are available for
a Stillbirth. Complete the Birth area if a certified photographic print of the
registration of Stillbirth is required. Complete the Death area if a medical
certificate of a Stillbirth is required.
Certified Photographic Prints can only be issued to:
Birth
the (adult) person whose name is on the birth registration
a person who is shown on the registration as the mother or father
the next-of-kin* for immigration and/or citizenship purposes
the next-of-kin*, a legal representative for court purposes (i.e. divorce,
estate matters, custody hearings, legal change of name)
any family member for the purpose of genealogical research
a person who needs it for the Child Welfare Act or Part 2 of the
Maintenance and Recovery Act
a person with a court order from a judge (attach documents to application)
Marriage
a party to the marriage
a legal representative of a party to the marriage
a person with a court order from a judge (attach documents to application)
Death/Stillbirth
any family member for the purpose of genealogical research
a person with a court order from a judge (attach documents to application)
a person who satisfies the Director that they require it to accompany an
application to disinter a body made under this act
a person for court purposes as required by next-of-kin*, a legal
representative of the court (i.e. estate or probate matters)
a person for life insurance purposes as required by beneficiaries, insurers
or their legal representative
Medical Certificate or Death/Stillbirth
Cause of death is restricted to adult next-of-kin*
Proof of relationship is required. (If a birth certificate is supplied it must
show parentage.)
*Next-of-kin as defined in the Fatalities Inquiries Act are:
Mother, Father, Son, Daughter, Brother, Sister, Spouse or Common-Law
Spouse.
